Ability to add a note when updating document templates

When updating a document template it would be very handy to have a note box allowing us to say what updates have been made.  This way we can see what changes were made and when and why.

Obviously we can keep this information in a spreadsheet elsewhere but updating multiple spreadsheets is a bit of a pain and will most likely get missed.

  this would be very usefull for providing info on a borderaux to insurers
