Record introducer reference when brokers are created

When brokers are created it might be useful to have a free format (10 character) box to enter an optional 'introducer reference' that would be shown on all reports relating to that broker or indeed cases created by that broker in future.

Note: The value in the box could also be passed in (development note: by querystring) so that third party websites could pass this value in when bringing new business to a scheme, allowing the return on investment to be more accurately reported than with web analytics.

  This feature has now been completed and is available on BETA for user review.  We have opted to use actual Agent accounts rather than freeformat box to achieve this.
