Add a broker fee per scheme

I'd like a simple way add a broker fee per scheme. We currently have to do this by adding the broker fee to the current policy fee on first premium & renewal ratings for each insurer on each scheme, meaning we currently update 38+ ratings each time we add a broker. 

This could be done through an additional option in the agent setup under the "Commissions & Fees" section.

  Yes, a default fee per broker would be useful.
